They didn't set up the forms with enough support and concrete is super heavy. Something like this: If you don't do your load calculations correctly and don't use enough supports, you're going to have a real bad day. Honestly, that could have gone so much worse, but ironically they did an excellent job with stringing the rebar.

It really would be an adventure getting that mixed right. belimo make ball valves with this weird orifice plate in them, that makes the flow response of a 1/4 turn ball valve a lot more linear. they're designed for automatic heating systems though, in the op's case just a pair of multi turn globe valves would be the way to go.
