Look up his other series, The Boys, for further examples. The main villain rapes and eats a baby, other bad guys smoke joints made from dried fetuses and so forth. It kind of becomes almost a case of trying too hard.

Also, the violence is mostly intended as black comedy; this should be fairly obvious given that the lead zombie in the pack chasing Ennis' protagonists around asserts his authority by beating his underlings around with a severed horse penis while yelling "HORSECOCK!".
