I've never heard of this. Typically if you eat a squid you're not swallowing the tentacles whole, you're eating chunks of them. If the tentacles are big enough to clog your throat you're not dieing because you ate squid, you're dieing because you don't know how to chew your food properly.

Oh yeah, I could see it happening with octopus arms as they are covered in much larger suction cups. In general, the suckers on the arms and tentacles of squid are much smaller.

Neither can I. I enjoy both octopus and squid, but they are EXTREMELY chewy. Even when cooked, it can take quite some time to chew through a large chunk. I can't imagine how difficult it would be to eat one live.

According to my friend who was stationed in Korea and his Korean mother, it's a common belief that eating seafood as raw as possible is better than eating it cooked because there are more nutrients. It's really popular with martial artists in S. Korea because eating a live octopus is seen as a rite of passage since you "fight" to put your food into your stomach.
