Well, his entire storyline certainly feels tacked on, but then again it's a total mess held together with chewing gum and a couple staples.

Hyrule Warriors is a dynasty warriors game with Zelda characters. It has costumes which have custom animation sets. It also has a bug where you can make costumes load on the wrong people.  e: awful.app

It's not really a glitch, but that reminds me of the moveswap mod/hack in Smash Bros. Brawl you can use to switch around the movesets of characters. Some bits of Runescape look really good, especially the f2p bits. On the other hand there's some bits that haven't been touched for years - and from what I understand, actually can't be because they're so badly coded that trying to change them will cause some kind of catastrophic failure.

It's more that they have no incentive to touch a lot of old areas. Most of them are gated in the Members areas, and people aren't becoming Members because the game looks pretty. So they focus on bringing in a boatload of new content for free and pay players, and that stuff inevitably ends up looking better.
