She starts the therapy session thinking that she is going to help Tony figure out how to be a better person, but all she ends up doing is helping him rationalize his behavior so that he doesn't have panic attacks anymore. Once she realizes that(or has it pointed out to her by her own therapist) she starts trying to extricate herself and drop Tony as a client but we all know how that went.

One of the most revealing scenes about how Tony has been using therapy is when he's convinced that Christopher wants him dead after seeing Cleaver. Melfi asks him something like "Without invalidating your feelings, is it possible you're reading into this too much?" and he responds "I've been coming here for years, I know too much about the subconscious now"

Better, possibly, but the Soprano crew was always inefficient as poo poo because Tony would lose his temper and kill someone he shouldn't or start some poo poo with New York. The only major earning loss he suffered that wasn't either directly or indirectly his fault was after Vito died and he lost his best earner.

He needed to talk to Carmela, Silvio and JT in order to realize that it was basically a revenge fantasy. Then he takes it a step further and convinces himself that it means Christopher is planning to have him killed and he uses his limited understanding of psychiatry to justify it.
