Look closely, when he pushes the second guy's arms upwards, he fires a shot downwards with the other hand. That's probably what drops the third guy.

Yeah, practical effects are simply not an option, there. That's why movies didn't use guns or violence of any kind until Industrial Light and Magic pioneered modern CGI.

As others said, there are a couple shots that my sloppily made gif didn't pick up. Also, say what you will about practical effects, but John Wick loving owned, get over it.
