This is probably standard procedure, despite the fact that to us it looks crazy as gently caress. In most industries working with equipment is all about letting things come to a complete stop before doing anything. That's not true about trains, rail workers are allowed (and probably required) to hook moving cars together and get on and off of slow moving cars. The only thing I see wrong with this is the speed of the incoming car is too fast.

Likely wasn't folded up properly (or it was and just happened to make an unexpectedly tall load when combined with the flatbed) and it smacked into the bridge.

It looks as if the flatbed snapped in half, too.

If you look at the lift, the basket appears to be above the boom. Perhaps that caught the bridge and threw the lift violently backward, breaking the flatbed?

This is roughly it. The standard rule is that you, personally, either control the incoming cars or make sure the ones behind you have the brakes applied in some way. Of course this goes out the window the second production suffers.

The company I work for has completely banned getting on or off moving equipment. Genesee and Wyoming owned shortlines.
