Akira Toriyama was so over Dragonball at that point that he really couldn't be bothered to take any sort of artistic stand with it. So his editor was basically like, "eh, I don't like 19 and 20 so much, replace them with something else" and they just immediately pivoted to 17 and 18 and threw in the necessary crap to tie up the loose ends from 19/20 and Dr. Gero. Then the editor goes, "I don't like 17 and 18 so much, you should replace them with something else" so they just made up Cell. And then Cell's transformations were entirely the result of his new editor being all "Cell's ugly, you should change that." If I'm not mistaken, Toriyama kinda hates Cell. I know he's acknowledged that the whole "we're gonna hold a fighting tournament to decide the fate of the world" plot was lazy as all hell and the result of his apathy toward DBZ.
