The squadron CO was flying with the guy in an attempt to reign him in, too. In higher-definition pictures of the plane taken just before impact, you can see that the XO managed to pull his handle, but he did it too late. The co-pilot's hatch visibly blew, the plane just managed to crash before the ejection sequence could complete. The picture in question (with the blown hatch clearly visible): Even if the seat had fired, he'd have almost definitely died, probably a lot worse than he did just riding it in. Said rear end in a top hat wasn't the guy who was taking his fini-flight, that was the WG/CV (who was in a jump seat as a safety observer). The WG/CV wasn't really in the loop on any of the decisions that allowed Holland to keep flying, and actually was a last minute add to the sortie in question so he didn't even get briefed on the planned flight profile (which was out of limits, as briefed.) The real lovely part is that the SQ/CC of the Bomb Squadron (who was the co-pilot) had been actively trying to get Holland grounded but had been stymied by his leadership (mostly the OG/CC, also the Wing King)...so he had put out an edict that no one from his Squadron would fly with Holland unless he was also in the plane (hence why he was on-board). His family also watched the crash from base housing. Regarding the nukes, Fairchild had been reassigned to AMC as a tanker-only base several months before the incident (the BUFF in question was one of the last ones to leave), so it is likely that the WSA was largely empty at the time of the incident.
