His theme song is called Doctor because it's a reference to the villain in Cave Story because the song is built on samples from the synth instruments in that game.
Personally I feel like John's actual theme song is Harlequin, because that's the haunting piano refrain he's playing in what was IIRC his first [S] page, and then Harlequin remixes were used for various strife scenes involving him.
