In the novels, none of Kyon's dialogue is ever given quotation marks iirc, so the difference between narration and what he says wasn't always obvious. There's a few times in S1 too I think where Haruhi seems to respond to something he just thought and that's an intentional nod to that quirk of the novels.

Having read part of the manga adaptation of the post Disappearance novels now, I want to say it's a real shame the anime never moved past that point, right now we're at the stage where the characters are "growing, growing, growing...", how some of them are struggling with old tendencies and in some cases slipping into even worse ones for a bit, without getting any of the moments from the later stuff that really show just how much they've grown over the course of it. Especially Haruhi, Sigh doesn't show it off too much because part of the point of it is her megalomania being exacerbated by getting in the director's seat, but she really is mellowing out and becoming a more decent person.
