Fiancee used to work at a hospital as a pediatric dietitian. SNAP and WIC were in many cases the only thing keeping kids alive. Even then you'd have kids coming in for failure to thrive or really nasty complications because even with SNAP and WIC their parents just straight up couldn't afford to get enough calories into them without resorting to foods that would interact with medical conditions. Cheap food is carb-heavy and absolutely wrecks the systems of diabetic kids.

Cutting SNAP and WIC straight up kills people.

One of the conclusions is that they need to get more doctors asking patients about food insecurity and pushing solutions; they found that a lot of doctors who had been treating patients over a period of years didn't know their status and thus couldn't help with it. Saying "you need to lose weight" is perfectly correct, and a pamphlet on diet and exercise is well and good, but what they need is stuff focused on getting eligible people in SNAP and info on how to use SNAP to make the best decisions out of the choices available.

So yes, hospital incentives are part of it

Pretty sure social working is outside the scope of a hospital's mission. Traditionally this is arresting imminent death, not warding off looming death.

What you need are more social workers that go into people's homes to follow up and poo poo, which requires a different bureaucracy entirely.

That's the thing - hospitals DO go further than just stabilizing patients. They do stuff like working with the food banks in the area to get them prioritized for fresh vegetables and meat and working with companies like Abbott Labs' charity departments to get discounted or free medical foods (which SNAP and WIC often don't cover) along with scheduling frequent appointments with dietitians or other specialists for follow up to tweak meal plans to what the family can do.

Of course that all goes to poo poo because the food pantries and charities are maxed out already, companies will only do so much, and the families can't go to the follow ups because TX hasn't bothered to expand medicaid and the people can't afford the $150/appointment expense and waiting lists for sliding scale treatment is a year long. So the family eventually gives the kid their normal dinner because the choice is between starvation and something that is unhealthy.
