I think the worst case I ever saw was a game where the girl's vocal tic was saying 'pya' randomly. Japan is the master race for running poo poo into the ground.

Medical eye patches are common enough in Japanese games as a cosmetic thing that after going "what, why, who would put this on their character?", I remember that it's in a bunch of other games and move on.

I remember when I was actually in Japan I read a good interview by some anime producer or character designer, and he was lamenting how the trend at the time was that every anime had to have a very similar set of character designs/personality types in it. He called it something like 'database design' and talked about why he thought things had gone that way. It was interesting and I'm really pissed off I can't find it now! If I can find it I'll post it, maybe it's interesting.
