Nah the artists draw an outline first with sharpie or surgical marker and they tattoo over that. Plus since just a line I doubt it took longer than 20-30 min per person.

Speaking of outlines how in the gently caress did this happen? Is it a horrifically hosed up "coverup" of an old tattoo?

1) I don't think it's an apostrophe, I think it's a back mole, like the one to the right of F.
2) The first sentence doesn't make sense unless it was "Miley's what's good". No matter how I read it the way you've got it, it reads as a question

It's referencing two things: Drake's album 'if you're reading this it's too late' which has that weird font and no punctuation in the title; and Nicki Minaj's comment to Miley Cyrus at the MTV (?) awards show last year. It's a terrible tattoo but the design isn't just something random.
