This entire conversation has made me extremely thirsty for a "general war never broke out in early 20th century Europe but some other poo poo happened and now everyone's nervous again" HoI4 mod. Though I guess that it's a lot more difficult to construct a scenario there that's both plausible and produces fun gameplay, compared to something like Kaiserreich. I think it's a lot more interesting of a question in terms of alt histories though, WW2 is so much the child of WW1 that it's intriguing to think what could have happened without it.

Would probably be some kind of conflict that looks a lot like WW1 politically fought with newer weapons. However you could have some fun working from the assumption that essentially nobody's fought an actual war since 1870 and this means that you haven't really got any people who know how to use those weapons properly since you haven't had anything like WW1 to kick you in the balls and force you to innovate. So I guess you could have some stuff like really large battleships, weird or lacking tank designs and maybe something like a lock on researching modern military doctrines until a war actually breaks out. It's really hard to imagine a successful communist revolution or nazism without WW1. Without WW1 its quite likely that the imperialist powers will have covered even more of the world without the world wars coming along to sound the death knell of the era of colonial imperialism, might even have something like an almost completely carved up China. Ottoman Empire might be around, since there hasn't been a major war, or the great powers could have found a way to partition it peacefully in the interest of avoiding war. Some kind of world system where Britain, France and Russia are essentially settling points of dispute between them by carving up the world between them and shutting out younger and ambitious powers like Germany, Italy and Japan could be a thing but might produce a situation a bit too similar to how WW2 turned out in history. Perhaps have Germany and Japan trying to gain an advantage against Britain by supporting nationalist movements in their colonies, if you still have the Ottoman Empire in some form then Germany might have gotten a lot further along in developing their infrastructure and strengthening their army, giving them a very real economic presence in the Middle East. The USA would also have been much weaker compared to the rest of Europe (simply because Europe hadn't trashed itself in WW1) which would make for a drastically different ww2 even if all the players were the same.

I think I'd like to see a HOI4 alt history that was more like Europa Gooniversalis. Split up into more balanced states that see warfare in all sorts of non traditional spots. Paradox game themed, so... Byzantium, Norse Vinland, Aztecs, Al Andlus, Old Great Bulgaria mixed up with Imperial Britain, the North German Federation, Italy, and Zulu with fighter planes. Like, just goofy and fun without worrying about a profound or plausible backatory.

Back in the LP Kabal there was talk of making a Clash of LPs scenario, Jerusalem Vs Texas Vs Ghana Vs Hohengermany Vs Byzantium Vs Neubauten Vs Gothia Vs Afghanistan Vs Scandinavia, with the rest of the world populated with allies and enemies of said nations, but making scenarios for Hearts of Iron 4 seems like a pain in the rear end in general.
