https://www.reddit.com/r/OSHA/comments/4thvle/recently_had_a_new_nitrogen_vent_installedright/quote:So we just realized we had this problem because our oxygen deficiency monitors (ODM) went off but none of our other systems showed any kind of leak. In fact, the sensors were reading a 25% oxygen concentration, when the normal alarm concentration is about 19.5%. There's nothing in our lab that should be generating oxygen, so we put our heads together and realized that the calibration sensors must have been malfunctioning. Those calibration sensors are positioned outside (bottom left-ish in the picture) and sample the fresh air about every two hours, generating a baseline for the ODMs inside. Unfortunately, they calibrated the system while liquid nitrogen was pouring out of the vent, so the baseline got a bit skewed.

Even the smaller crane smashed a guardrail and punched a hole in the deck. Traffic is going to be hosed for a long time up there.

The tappan zee is also where the boat crashed into a crane barge and four of the crew died. That construction project is immense, but seems to end up in the news quite a lot.

It looks like it's supposed to be a mat slide, but it's disused so there's no cushioning at the bottom and the guy's basketball shorts have incredibly little friction so he just goes flying off.

that slide video makes me want to re-watch the clip of the guy hitting his tailbone on a curb in a Wal-Mart parking lot. I think he was riding a shopping cart or something.

There's a line the original document that says "an organic absorbent" should be used meaning something that can absorb organic material. The inorganic clay worked well but some idiot who knew nothing about the actual chemical reasoning read it and said WE NEED TO SWITCH TO AN ORGANIC BASED ABSORBENT ACCORDING TO THIS SENTENCE because putting organic material and nitrates together with heat is not the stupidest loving idea ever.

Humphreys posted:Fresh from Facebook! Looks like a fairly typical 'emergency patch' type of thing after the line was cut by a trencher who probably didn't have the lines properly located before digging. Depending on what's in that line, they may dig out a larger pit and do a permanent repair later, or if it's just water or something they'll probably just leave it like that. If it's gas, they'll come back and replace an entire section of line since they'll want to get rid of the whole section that got stressed by the hit, but that becomes a non-emergency repair so it's typically done by scheduled crews instead of the emergency response guys on a small line like that. Of course, this is only true in the area I used to work, who knows wherever this is.

Re: the recent arresting cable snap on an aircraft carrier: I'm pretty sure "flight deck crewman on a carrier" is right up there with "unarmed black kid stopped by police" in the running for odds of horrible death. I don't think this oldie but goodie has been posted in the thread: https://www.youtube.com/watch?v=5jxcSY1AwrM The guy survived with non-life-theatening injuries (broken collarbone, got hell of roughed up, but was talking on TV the next day) -- he got wedged in the intake channel, and the big explosions you see there are the engine digesting his helmet, finding it unpalatable, and throwing all the spinny-knifey-bits out the back, andthe pilot immediately shut it down. https://www.youtube.com/watch?v=GF3Iz7b95-8&t=48s Airliner turbofans with the big intakes and the blades RIGHT THERE aren't so forgiving, get too close to one of those, it spits cooked hamburger out the back, and you're identified by failing to clock out at the end of the shift.

As opposed to this guy, because unless you're lucky enough for your cranial to FOD the engine to death, you'll do it, and with a large jet engine that's like being crammed into a Vitamix.
