Is there any type of cross realm grouping? clans/guilds? My friend and I have characters on different servers, wondering if that will cause an issue.

All PvE is on megaservers now, people from all servers in a world region (NA, EU, is there an Asia region?) just get all lumped together in the various PvE map instances. As mentioned, the only thing your server determines is who you can play world vs world (the large-scale PvP mode) with.
