My only experience with Uber was in San Francisco where it was great. The fact that Vancouver is still banning it probably means it's better than taxis, which are prohibitively expensive and try to do things like bullying my wife into paying with cash - even driving her to an ATM in the middle of the night against her will.

Exactly the same as Telus and Koodo. All three share a network. I believe it's technically possible for a carrier to prioritize customers on different brands but I've never noticed this happening.

It's fuckin 2.3 million dollars to go 5 feet here in Newfoundland and all the cabbies are up against it here too. I used to live honestly 1-2 minutes from the airport, grabbed a cab home from a flight once, ride was $13 which was already ridiculous. I hand over a 20 and say $5 change is good, "actually" the guy says and points to a sign on the dash that they add a $5 charge to resedential addresses from the airport for previously unexplained reasons. loving rogues.

Its a program (or multiple ones) that understands the algorithm to generate the code from the phone's IMEI, like keygens are for software. The average person would have to pay for one, I've seen them being sold for several hundred dollars. Giving these away would cost all these businesses a lot of money, they're too profitable to give away even at $5 per customer and the people reverse-engineering the software on phones to figure them out work too hard to give it away.
