Well, all major plays in New York are officially filmed for archival purposes, but I'm not sure what the actual quality is besides "camera on balcony". A big issue is logistics, some plays only run for fifty or a hundred performances, or the producers balk at putting out a recording a performance that might diminish interest, or it might just be too expensive for whatever reason (a lot of plays and musicals run at a loss to begin with). Mostly I think it's just pigheaded old-fashionedness.

Ager IMO has really ordinary opinions, he's just not afraid to talk about mis en scene the way characters are dressed and things like that, which for some reason are seen as uninteresting or unimportant for things people spend months editing, scouting out, planning and so on. As far out as he gets is something like the Ludovico Lie, which is tremendous discussion on how Alex deLarge's treatment actually "works" and what it means.

I think rights probably have a lot to do with it. When a producer acquires stage play rights I am making an educated guess that they license the rights to stage a play in a theatre only. To record and exploit those rights commercially would require more payment and could also conflict with any film/tv licenses granted on the same property, which are almost always exclusive.
