Adding to everything that was said before; Republicans don't hate healthcare itself. They just don't believe that government should be involved in any way, either through subsidies like Medicare, or mandated (mandatory health care or you have to pay a tax penalty). Capital R Republicans believe in free market health care which leads to things like uninsurable (high risk) people who have preexisting conditions. It also might mean that Republican care could be less expensive (market forces dive down costs due to competition) or more expensive (no pool of low risk individuals subsidizing high risk individuals) One big election LOL is that we have 2 kinds of Republicans in charge, Ryan Republicans who would prefer no government involvement, and Donald whose promises require a lot of government involvement, and they are diametrically opposed.

The ACA was just a market study for the insurance companies at this point. They got all this great data out of it and now they can figure out the absolute best way to maximize enrollments while providing as little service as possible (open markets, separate plans for pre-existing conditions etc...). The only reason they started pulling plans out of the market place early was because they had all the info they needed on it. They fully expected to eat it big in those first few years.
