The reason the trains shut down a lot in NYC is because, well....living around the trains should breed a healthy respect, but instead seems to create hubris, and people wander down onto the tracks, or fall while drunk, etc etc. This happened in my nabe this Sunday: http://gothamist.com/2017/04/03/subway_fatal_r_train.php

Less sad Chicago-area train-freude: Driver Said GPS Led Him To Elevated Tracks, Collision With Metra Train To clarify how this could happen, in some of the suburbs, the "elevated" tracks are accessible from the road from a very steep hill over the tracks and usually roads that are closely parallel on one or both sides. No one died, but Grandpa probably doesn't drive anymore.

One instance when I was in rail network control my whole line was stopped because of a disgrutled ex-employee laid traps across a few sections. The traps were simply screwing with the signal voltage across RTC controlled sections of track which blew up my console telling me there was an engine where it shouldn't be and out of rotation. That cheeky fucker held up a few hundred KMs worth of my 'line' for a while. To note - sections could be between 1km and x hundreds on freight routes. Finding where in the section the fuckwit put the tripping device was a fun one for local track crews.
