Cora 'died' and fell through the world, preventing me from interacting with her to revive her. I got stuck with her and Drack in my 'party' even in areas where they shouldn't be, thus the dupli-Drack in this video. Restarting the game fixed it, but it persisted all over the place until I did that. Getting out of the Nomad would spawn a 'dead' non-interactable Cora somewhere nearby.
