So you can't credit existing medicaid/medicare/obamacare spending to that number: it would be $32t on top of that. However, a good chunk of that is, as people correctly assume, everyone else's spending on heath care that would be shifted to the federal government. However, there would still be a considerable net increase in health care spending overall by about $6.6 trillion over the same time period (2017-2026) as the $32t number. I'm pretty sure the $32 trillion source is this: http://www.urban.org/sites/default/files/alfresco/publication-pdfs/2000785-The-Sanders-Single-Payer-Health-Care-Plan.pdf So, $6.6t of new spending, ~$25.4t of redirected spending (state, local, personal) that is now paid for by the federal government.

Giving those 25 million people healthcare costs money. Sure, there are savings elsewhere, but the care for that 25 million isn't free.
