For the sake of my sanity, I'm going to assume that the few Establishment Republicans remaining in his administration will make it very clear to him how bad that would be and prevent him from doing it. See: Secretary of Agriculture Sonny Perdue convincing Trump not to leave NAFTA by explaining to him how bad that would be for Trump Country.

What would be necessary, in practice, for the payments to stop? Is it possible that "administrative delays" could have them continue until congress passes appropriations, or is it more like one tweet and they're gone?

Yeah, insurers have to submit premiums to regulators well in advance. Even if something is worked out, they have to assume it won't be.

If premiums are set high and subsidies added later, I think people would get reimbursed under the 80/20 rule that says insurers must pay out 80 cents of every dollar collected or rebate the money back to members.

Except instead of firing him properly Priebus was unceremoniously escorted out of AF1 into a private car and told by some other staffer that he had been axed. Trump didn't do the deed himself. He's a coward.
