Going back to this fun little surprise, any info on it? I'd guess there's a short to ground from one of the secondary conductors that catches on some oil that then blows the top of the terminals off the can, but not sure.

Looks like he just finished working on whatever he was doing and when he went to reconnect that breaker with his deadman pole he realized very quickly that his truck keys were still hanging on the line. What I have seen in a lot of places are trapped-key interlock switches. An example of that is a big robot in a cell. The power supply for the robot has an interlock key. In order to unlock the caged area the robot is in, you need to switch off the power feed, which mechanically releases a captive key that you then use to open the cage. While the cage is unlocked you cannot turn the power feed back on without they key.
