Going back to this fun little surprise, any info on it? I'd guess there's a short to ground from one of the secondary conductors that catches on some oil that then blows the top of the terminals off the can, but not sure.

Looks like he just finished working on whatever he was doing and when he went to reconnect that breaker with his deadman pole he realized very quickly that his truck keys were still hanging on the line.

One thing I've learnt from this thread and IAEA reports into radiation incidents, is that no matter how elaborate and difficult to overcome the interlock safety system may be, humans will eventually circumvent them to save time and effort.
