is state-level single payer something that can work? I've always felt that it needs to be a nation-wide federally administered program given the governments more broad authority to tax - especially with the medicare payroll tax

Vermont probably could have done it too. Anyway, there's a bill in the state legislature now to do Single Payer Primary Care, which would be significantly less expensive and offer obvious benefits.

also MfA is kinda poo poo if the bill is just "minimum age is zero END OF BILL", medicare needs some real cost reforms like for example not make it explicitly illegal for medicare authorities to say "we don't want to pay that much for that pill"

there's nothing that says the fees the government pays out can't vary to meet local market prices, but you're right that simply nationalizing the institutions would be easier

re nationalization: doctors should make like 60k on 20 hour work weeks and there should be 10x as many of them . since residency slots are paid for by Medicaid this isn't a problem
