It would take the manager, the player himself, the other player and any player on the pitch with some semblance of sportsmanship to not alert the referee in this case. Plus all of the refs would have to also make the mistake. I don't think this has happened in a long time, certainly not in the VAR era. In Serie A, where VAR was active all season, the VAR refs would actually review cards as they happened even if there was no action to be taken.

If there's any challenge at all play would stop. That said I can still see this happening.
