Heroin doesn't really make you lose weight at all. It's more that being a heroin addict engenders a lifestyle often incompatible with getting three squares a day in. So if you got the money and the ease of access that you can just do heroin and eat like a king it'd be simple to gain weight.

But as a woman you sort of have to pick one and there's obvious upsides and downsides to both. The wife is a public prestige product and low weight is a component of that, the mistress is there to be a fun good gently caress and can fit a man's private preferences.
