Assuming the scaffold is rated to handle the fall loading to use as a tie down point the biggest danger is probably them moving the bits of floor withing having it tied down, but depending on what they're working over letting it fall might be the safer option anyways.

I don't know how often elevators are SUPPOSED to be inspected, but this one apparently hasn't been in my lifetime. Also, someone misunderestimated their vehicle height. I don't know how they mnaged, my car could barely fit into the drivethru.

In my province they have the expiration date printed right on it, and they need to be renewed annually. Sometimes an expired one is on there for a month or two, since there's only one inspector for the entire province. Even then the permit sign in my building gets vandalized if it stays expired for too long. Of course, Alberta seems to hate any and all regulation that gets in the way of big business, so I guess I can't be too surprised.

Inspections aren't yearly and there is always enough of a tipoff to magically replace all the tags before inspection with a fake expiration date. Also a warehouse that never has all the lights on because if they all get turned on - they trip the breakers. This is the warehouse specifically for our Electrical branch of the company. If only there were skilled qualified people around that could fix the issue, with parts in the warehouse.
