Definitely an old tattoo. He's probably getting it covered up and the artist wanted to do that for a laugh before he fixed that mess.

To get even more  about it, shaving cream is not and shouldn't ever be used in the tattooing process, and the guy isn't wearing gloves. There's no welting/swelling of the area. If the print-out was used as reference it'd be covered in ink smudges, and even without the close-up a lot of the lines and solid black areas you can see where the ink has fallen out after healing.If there is a cover-up image out there I'd love to see how/if this was remedied.

No and you're lucky if they even wet the cheap disposable razor before they go to town on you  I can't imagine the mess that would occur in your average tattoo place if everyone started using shaving cream and gels to shave people for tattoos.
