I am absolutely lost as for what's going on. So it seems like there's at least two head doctors running around at the same time. Could be Tsurugi's doing. But whoever is disguised, he/she can also attack by making them run into stuff... And that is somehow bad enough that it pretty much severs someone's legs? Is this some kind of weaponized bad luck stand (a bit like Ticket to Ride), that can retroactively make any tiny misfortune worse and worse over time? (So a small scratch turns into a giant wound, etc.) That doesn't explain what seems like a second doctor, and teleportation; but it DOES appear that the doctor at the estate and at the hospital have the same powers, because the wife also got hurt in a minor way trying to go after the doctor. Sooooo... - there's at least two of him who appear to have the same powers - can make those chasing after him suffer accidents - those accidents turn much, much worse eventually (???) - he might not even be real If Kaato is involved, that might also be how the kid got crushed by the gate. Kaato shows herself to the kid, who had already teased Tsurugi about his grandmother the child killer. Kaato (or whoever???) puts kid under the effect of the misfortune stand. Kid chases after grandma, because HOLY poo poo THE ACTUAL KILLER. That counts as chasing after her, the stand activates, gate closes. Before that, Tsurugi had put his family under Paper Moon King's effect so that they'd see Kaato as the doctor. I have no loving clue.
