Which doesn't mean that different systems, policies, people, can't mitigate or exacerbate the risks. But that it's not something that anyone, ever, has been able to excise from the human condition.

Speaking of which, does anyone know good literature about other major manmade catastrophes caused by a combination of human corruption, shortsightedness, and hubristic attempts to manipulate a ridiculously complicated system leading to cascading consequences? I'm already reading about the 2008 Financial Crisis, as well this show. I'm thinking I want to read something about China's Great Leap Forward, or maybe the original Great Depression, or some other horrible societal cataclysm I can't think of right now.

It was a FOX series that took the X-Files formula and applied it to fringe science. Five series, with the fifth one commissioned rather obviously to get it up to the magic 100 episodes. It had John Noble in it, whos always a delight to watch, as the lead character, and Harris played the Big Bad of series 1. It was really enjoyable and was a cult hit, which explains why it survived half of its run in the infamous Friday night death slot. e: I actually looked it up and Fringe was basically the last show to actually run in the FNDS before Fox decided not to put scripted drama in there any more. The large red heap beneath the reactor is the primary part of the molten fuel that has melted through the bottom of the reactor chamber. I believe the smaller amounts underneath the main heap are in the water tanks they drained, but the elephant's foot is the main goop at the top.
