It's a less meta version of GTA V being about s guy coming to terms with being a GTA protag via midlife crisis, just Max is a guy who is very good at exactly one thing and tht thing is awful and has brought untold pain into his life and he finally embraces it and tries to at least stop people from causing other people hurt Like that's what the hospital level is
