Not only do Americans miss out on public health care, but many lack any adequate paid sick leave (or it requires a doctor's note) and most Americans are living paycheck to paycheck. If this virus does require people to stay home to avoid the spread, that's not going to happen in America. The population can't afford it.

"There is a possibility that it is contagious during incubation" would be much different than "it is contagious during incubation".

Acting like it's fact without any given evidence leads to things like entire cities being shut down and people in a constant state of suspicious fear over whether or not they or anyone around them could be sick. Not sharing fairly groundbreaking facts with any other health organisations around the world is deeply irresponsible as well as pretty suspicious.

Assumptions like that are not neutral and they do have negative consequences. If it's not true (or if they don't have drat good evidence that it is at least a good possibility), it's going to seriously impede efforts to track down legitimate cases because suddenly field workers need to track down vast swathes of a population. Case histories are a nightmare when you have to try to pinpoint even a single person's movements for an entire two week period and follow up with every single person they came into contact with. It slows down the very important field work necessary to track what a virus is doing and how it is impacting a population. That's before you even get to things like mass quarantines, panic, overcrowding in hospitals and clinics from scared people who no longer have any idea if they could be contagious or sick, etc.

The spread of information is critical during any kind of public health crisis, but that information needs to be accurate, and people need to be able to have confidence in what public officials tell them.

"virus" covers an enormous range of different pathogens, and different families of virus behave very differently. One group of viruses commonly being contagious during the incubation period isn't that relevant to another group of viruses that have never previously been contagious during incubation. It's similar to how some viruses can spread through the air, while others require contact with bodily fluids. They don't all just mutate to have the same traits.

If they have evidence that a coronavirus has mutated to the point that it is now contagious during incubation period, that is really huge, and it's really important for everyone working in epidemiology to know the details ASAP, not some public statement without sharing any details with anyone else. Right now, there's a lot of people saying a lot of things because everyone is desperate for information, but accurate information takes time.

It *could* be true, but it's a really huge claim to make without any further explanation. A more cynical explanation would be that saying people are infectious for up to two weeks before showing symptoms gives the Chinese government enormous scope in limiting freedom of movement and shutting down access to services across the country.

Thanks for the good post.

Got any info on incubation lengths for other corona viruses? How out of the ordinary is 14 days?

Here's a political hot take, and forgive me if I'm a loving idiot. The lack of public healthcare in America actually stymies the spread of the disease, as people who are too afraid of bankrupting themselves do not go to the hospital. They die in solitude, causing the coronavirus to peter out in the USA. In countries where healthcare is guaranteed, the hospitals are flooded with people, some are not even sick but pick up the virus while waiting thus causing the virus to spread like wildfire and wipe out their populations.

14 days would be an exceptional incubation period for the population at large. Incubation rates will always vary to some degree, and there will be a small proportion of people whose incubation period ends up much longer than usual, but most people will have much shorter periods.

The types of coronavirus that typically infect humans have an incubation period of 2-5 days for the more common ones, 2-7 for ones like SARS and MERS (which is the nastiest one, also still active). It should be emphasised that the current incubation period of this coronavirus is still unknown - they seem to be going with worst case MERS incubation as far as I can tell.

There are 6 other coronaviruses that infect humans. Two of them jumped from animals to humans - SARS and MERS. SARS incubation was usually ~5 days (so the incubation period for some was 3, most were 4-6, some at 7 and sharply tapering off past that point), but a statistical minority of cases had an incubation of 10+ days. MERS is the same, but there were two patients tracked who had an incubation period of 9-12 days. Most people with anything other than mild MERS are hospitalised within 4 days though.

More likely folks would do exactly what they currently do which is only go in to the emergency room well past the point where they would have if paying for it weren't an issue, resulting in more people being exposed and higher morbidity.

Meanwhile, conservatives will chortle about how those people are just so irresponsible, because clearly their reticence about going in for a check-up sooner was a personal failing on their part and not a result of systemic inequality and poverty.
