As the article hints at, the case workers at the Quebec division of the Parole Board were inexperienced, apparently because they were all replaced (just about after the liberals took over, iirc, but don't take my word on this) except for two of them. "Junior" parole board advisors are not supposed to handle cases unattended without a mentor, but because there were only two left with experience, that's precisely what happened. Now, the real question is what happened that they'd decide to replace 14/16 parole board members in one swoop, and where the poo poo did they find these clownshoes that not only failed to recognise this guy as a high risk recidivist but also thought it'd be a good idea to actively facilitate breaking the law by use of sex workers. That's what the inquiry will find out, hopefully, but what the Christ.
