The disparity between exit polls and vote totals in 2004, favoring Bush over Kerry every time, largely came down to states using optical scanners for paper ballots. You can rig the scanners, obviously. Although at least there's a paper trail that can be audited (none of them were because Democrats are pushovers that like losing). I think it is possible to electronically tally votes, but only if people are okay with ditching anonymity. You could have a public database of people's votes, and then at least everyone could audit their own vote to see if it was changed.
