Log082 posted:Anyone have good recommendations for good OSHA books, preferably those that do a deep dive and analysis of a single industrial accident or wreck? I prefer the type that puts it in a historical context rather than just technical descriptions of what went wrong. It may not be 100% on point for the OSHA thread, but Shadow Divers is a good read. It's about people diving an extremely dangerous wreck at record breaking depths. They were basically blackout drunk for the few minutes they could stay at the depths they were driving from Nitrogen Narcosis. (Nitrogen narcosis starts at 100ft deep, roughly. They were driving at 230'-240') Oh, and they later started experimenting, as "hobbyists," with new gas mixtures. You don't need to know a whole lot about diving, or just living tbh, to know that "loving around with gas mixtures and creating your own dive tables" is real cowboy poo poo. These dudes were reading US Navy studies from like the 50's and trying to replicate them for "recreational" dives. 3 people died 2 marriages ended because of the obsession It took 7 years to identify the wreck of a U-Boat 60 miles off the US shore that both Americans and Germans denied the existence of. It likely sunk itself https://en.wikipedia.org/wiki/German_submarine_U-869

Regular old SCUBA regulator. And has been said, yep, super common for panicked people to rip off mask, reg, hood, other people's poo poo... It's scary as gently caress to see it happen and be basically unable to help. The best I've managed is to grab their octo, press the purge button, and jam it onto their upper lip at an angle. They're gasping for air, underwater... the best you can do is try to provide some actual air as they speed to the surface. If they take the reg, let go of the button or you'll pop their lungs. It's hard to tell the depth, but it looks like they come up in a big hurry. That's an immediate evacuate to the beach and call DAN for recompression for everyone, IMHO. An air embolism is a real possibility if the diver held their breath, so is secondary drowning, or getting bent.

Not sure if it's been mentioned recently, but Command and Control by Eric Schlosser is a great deep dive/dramatization of a Titan II explosion precipitated by a dropped wrench. Between the story about the Titan II incident he goes into how we try to keep nukes from accidentally going off and whatnot. It's a good read.

Atomic Accidents by Jim Mahaffey is also a good book on nuclear safety, but primarily focused on the nuclear power industry. Lots of anecdotes about criticalities in waste reprocessing plants where someone decided to take short-cuts. "So we could use the proper tool for stirring all this uranium oxide in solution. Or we could just grab a fifteen gallon mixer and dump a whole bunch in!"

I have both Command and Control and Atomic Accidents and highly recommend both. Atomic Accidents is particularly good for events that most people (including me, before I read it) haven't heard of.

In addition to the nitrogen narcosis, they were at depths were they would have maybe 20 minutes of time on the wreck and then have to spend hours decompressing. All of this to go into an unexplored wreck, when wreck diving (even in well known wrecks) is one of the most dangerous types of diving you can do. One of my favorite little bits from the book that's very on-brand for the thread is one of the divers making his own gas mixes in his garage by operating the valves on the compressor with his left hand through the window. His reasoning was that if something went wrong and the whole thing exploded, the wall of the garage would shield his body from the blast and he'd only lose his non-dominant hand.

I feel like there’s got to be a better way to minimise risk here. Also, unless this is like a serious masonry wall, the pressure vessel won’t give a gently caress.

There are far worse things than goatse to pass around. I've seen far, far too many atrocities on the internet. They can try to soften the blow with hundreds of words of text like your link, but you can literally listen to his last gaspses of air, and watch his body recovery, if you want. I don't recommend it. That sort of thing piles up and doesn't go away. There are things you can't un-see.

Similar story from inventor Gaston Glock. He shot the prototypes one handed with his left hand for the same reason. That operator is an artist. I've never seen a wrecking ball wind up a haymaker swing like that.

The Station nightclub fire video is what I wish I could un-see. The person holding the camera is standing out the front near the sunroom and you can hear the screams of the people trapped inside (who never made it out). All because the venue owners didn't install the required sprinkler system (and the bouncer who didn't help matters by refusing to let people leave via the stage exit because that was "for the band only").
