What happens if there is a second car going the other way? Does one just drive in reverse til you hit a spot where you can pass? Is there a signal at each end to say "someone is already on here going the other way."? How does this work.

Typically right of way goes to the vehicle going uphill. On flat then to the larger vehicle or if they are the same then to the one who must reverse the most to get out of the way.
