school starts in 1.5 weeks. colleges start in 3 weeks.

what's keeping the hospitalizations and deaths down is that the state's population is old with fewer children and has the highest vaccination rate. they refuse to disclose the breakthrough rate on new cases, but 25% of the hospitalizations are fully vaccinated, sooooo......

you can get pretty far with a high vaccination rate, low population, extremely low density, and most activities for tourists happening entirely outdoors, but only so far.

Hospitals in Texas are warning of strained resources during a week in which more than 10,000 coronavirus patients have been admitted to hospitals in the state.

At least 53 Texas hospitals had intensive care units that were at maximum capacity. Two in Houston have been so overwhelmed that officials ordered overflow tents to be erected outside. In Austin, intensive care units were running short of beds. And in San Antonio, virus cases reached alarming levels not seen in months, with infants as young as 2 months tethered to supplemental oxygen.

“If this continues, and I have no reason to believe that it will not, there is no way my hospital is going to be able to handle this. There is no way the region is going to be able to handle this,” Dr. Esmaeil Porsa, president and chief executive of the Harris Health System, in Houston, told state legislators on Tuesday. “I am one of those people that always sees the glass half-full, I always see the silver lining. But I am frightened by what is coming.”

I think the waning efficacy stuff is really unclear right now. A lot of the waning efficacy may be the fact that we're also dealing with a variant where the vaccine is simply less effective even if you're in best period. But its really slicing hairs. I don't think you should believe that any vaccine (mRNA or otherwise) is going to be totally effective regardless of your booster status against the level of community spread right now.

I think what the science is going to show is that we have to target boosters to strains for serious immune response, but that's an arms race we're going to lose.

In my dumb opinion, some of the best evidence that boosters are required come from neutralization studies comparing convalescent+vaccinated plasma vs vaccinated-only plasma, for example this one and the associated plot:

The convalascent + vaccinated plasma neutralizing antibody levels are an order of higher in magnitude compared with the vaccinated plasma. The vaccinated-only plasma in some cases doesn't effectively or only marginally neutralizes the Beta+Delta variants, but the convalescent+vaccinated plasma basically neutralizes all variants effectively. The main point is that vaccine induced immunity can clearly be better because we see clearly better antibody immunity in the convalescent+vaccinated cases.

You're right it's not clear these neutralization studies translate into different clinical outcomes, but given that breakthrough cases don't seem particularly rare anymore it's not a bad bet that people are being impacted by only marginal antibodies levels. Additionally you're right that it's not clear a third dose of the current vaccines would actually improve the neutralizing antibody levels and more research is required, but would you really be surprised if that's what the research concludes? The whole point of booster shots for other diseases is to boost antibody levels and obtain more durable immunity.

To turn it around you say we need a booster shot based on the delta variant to do much better against it, but here it seems like convalescent plasma + vaccine induced antibodies do just fine against the delta variant. Note this plasma was obtained from people infected before delta appeared.

Cobb County School District in Georgia issued a statement saying that they won't impose a mask mandate because, while they realize that it would be safer, that the anti-mask people seem crazier and it would be less damaging to piss them off than piss off pro-mask mandate people.

quote:

Sharon Abney, Gentile’s sister, who lives in East Cobb and is a physical therapist, said the data isn’t concerning to her.

“The kids, yeah, they’re gonna get it, but they’re probably going to be asymptomatic or have a really mild case,” she said. “There are people in our community who believe that because we’re choosing not to send our kids in a mask, we’re killing them. And that’s not what’s happening.”

I must not understand r2 right. I take it to mean "it explains this percent of the variability", so to me you get something as complex as covid vaccination choices and you can explain 44% of the variability, that's a whole lot of the variability right?
