ECMO, much like everything in the medical ICU, is intended to support the boDy in the hopes that it can heal itself. None of this stuff is curative (aside from antibiotics). Venoarterial ECMO It has the most success as a bridge therapy for lung transplants in adults. Due to expense and scarcity, it mostly gets used as a salvage treatment for acute resp failure in adults. But at that point they're usually too sick to heal and it's just a slow steady spiral.

If we got good candidates on ECMO earlier, outcomes would certainly improve. But its kept as measure of last resort, keeping outcomes bad thereby justifying admin decision to keep it as measure of last resort, etc
