If I had to guess on that Star Citizen bug, there's some model scaling happening. Somewhere in the hierarchy, there was probably a point where the assets were built to the wrong scale, so rather than fix their asset and pipeline they just put in a special case scalar so it "just works" in the game. This is probably fine in most instances, but here when you apply the outfits it forces a recalc of the model's geometry, which then recomputes its location. In most games this still isn't a problem, but Star Citizen is evidently still working with solar-system scale locations with floating point precision, so every physics recalculation has a small amount of error in it, and that error is multiplied by the distance the model is from 0,0,0 in the world, so every recalc can be off by, well, that much. I'll say that it's especially cool that the game demands "fidelity" and "realism", so they had to make this happen in the shared world rather than load a separate model in a graybox room and only apply the outfit once you are done playing dressup, and therefore you can glitch yourself out of your spaceship into the vacuum of space by putting on pants. 10/10 bug, best open development of 2014.

It's actually kinda funny for a long time the only check for vacuum was a "Kill the player if they're not wearing a space suit in the airlock" check when they hit the "Cycle Airlock button".
