I think that might just be a music sheet display of some kind. Edit: no. It's definitely an instrument of some kind but the only two I know of with cranks are portable organs (like organ grinders used) and the hurdy gurdy and that's neither.

I really wanted to be able to answer this but now I'm wondering the same thing. It looks like an elastic band that may be driving a mechanism underneath, or perhaps making noise from the ridges on the drum cylinder? Very much not a normal orchestral instrument afaik
