Considering how this season has been going in terms of callbacks, I wouldn't be surprised if it's not Renee who is important but Dr. Ramirez because Rios' infatuation with her seems similar to Kirk's with Edith Keeler. It might not be the case but that seems possible since Picard is in the messed up future and Dr. Ramirez might have had some impact like Gabriel Bell. It's also possible they saved her and that messed up the timeline, like Keeler, and she has to die

You are an odd fellow, but I must say... you throw a swell shindig.

I am becoming more convinced that Q’s gently caress up was creating a predestination paradox involving Picard and co creating the Confederate timeline in the first place. Like he’s going mad being trapped in an endless time loop somehow and at some point he just decided “gently caress it, I’ll just do a Terminator on Renee and stop JL and the Federation from existing in the first place.”

Wish we had more of that, this season.

Star Trek was never good at handling mental illness but I guess a two minute pep talk now cures depression, yikes.

I also don't understand the timeline of how Picard was saved, they were suddenly at the clinic. Considering the circumstances there is nothing to suggest they just beamed everyone over and even if we handwave it away and say they did then why is noone wondering where Jurati is?
And this is of course just one problem of many in this episode, not to mention the bizarre musical out of nowhere.
This show is just as bad as Discovery in finding its own tone. You want to be campy? Ok, be campy but then you can't be so dramatic and expect the audience to buy your speeches and highest possible stakes.

I don’t think this show even remotely takes itself as seriously as Discovery does. This is Star Trek: Scooby Doo, and it’s pretty consistently stayed in campy romp territory I feel.

Let's be honest, even the pep talk was very meh.
It also continues to be just weird how Picard's mother is suddenly something Picard can't stop talking about. It's just so poorly setup as a plot point. His mother is now super important to Picard because they decided she is but there was no inciting incident for Picard to have such a focus on here. A better show would have done SOME groundwork for that in S1 or at least bothered to create such an incident at the beginning of this season but instead it's just there... (for some reason Picard suddenly developed an unresolved trauma)

I'd hope they catch the motherfucker who was behind the wheel and prosecute him for (hopefully attempted) vehicular manslaughter and then make sure the first words I beam back to Earth from Europa are "gently caress Elon Musk."

E: Actually that would be hilarious if suddenly Actual Consequences ensued and the Europa mission was delayed for like 9 months because the mission's pilot had to testify as the star witness in the criminal trial of famous eugenicist billionaire Dr. Adam Soong. Q accomplishes his goal because the United States legal system is slow and it sucks.

Don't worry, since this is a TV show, it'll be resolved in a single courtroom battle where Dr. Soong admits to everything after Columbo solves the case.

I wrote this exactly because I have to deal with the same issue (and another family member that deals with schizophrenic depression which is even a lot worse) and I'm sure you should also know that it's a constant battle but the implication of the show is a rather simplistic take and it didn't earn any other reading. It also doesn't address the fact that it's obviously nonsensical to talk someone into such a mission who has depression and makes you also wonder how someone like her could even be on such a mission (mental health evaluations for this kind of job are taken seriously).
So maybe this point is nitpicky but it's just another thing on the pile of things that are just so surface level in this show. If you really care about such a topic then explore it properly and put some effort in your storytelling because else it just doesn't deserve more than a "bad faith reading".
Maybe it's a personal thing but it just really annoys me if mental health problems are used in such a lazy way (it contributes to a poor understanding of the issue in general). I doubt the writers even know what exactly Renee has outside of "generic mental health issue that can be just anything" which is always a convenient TV sickness solved by a few kind words.

This may or may not be correct.

Incredibly weird episode. Goofy enough to still be fun despite some nonsense plot details and minor bits of dressing, some of which Star Trek has done before, like Jurati talking to herself at a normal speaking volume in a crowded room and somehow no one notices, I feel like every previous series has done a version of that, most notably Ezri arguing with Joran on DS9, other odd moments probably would've felt at home in a comedy like Lower Decks and not so much in a drama, spotlight tech and live band just rolling with Jurati's karaoke and no one else not even venue security batting an eye at it.

I think I hate that song like Worf hated Irving Berlin. Years ago I worked a graveyard shift and the afternoon manager would leave the radio on when she left, with the controls locked away in the office, so none of us night crew goons could turn it off or change the station, and I'm almost positive that song or one of its covers was in the regular rotation for all three years I was there. Just goddamned maddening. Solid performance in this episode at least, Agnes got pipes

Anyway, the pep talk and some nice smaller moments with the various character pairings also helped elevate this episode a bit, especially Rios geeking out over real cigars and real matches. Definitely not one of the standout episodes this season but not bad.

Definitely agree with speculation upthread that the true divergence could be any one of like three or four things: Jurati getting Borg'd and wandering off, Adam Soong messing around with his Rei Ayanami collection with Q's backing, or Rios getting too close to Teresa, and Renee Picard just happens to be tangentially connected to those other possible inflection points. Renee was still Q's original target, but his loss of control and the La Sirena crew not exactly playing the scenario according to Starfleet standard has spun off the situation in a bunch of directions he didn't anticipate

Edit: I'm going to double down on this. Picard knows obscure historical trivia like Gary 7, and on TNG he talked about his ancestors multiple times, but he asks NotLariss "tell me about Renee." I think Q is pulling things like Rene's name, Lariss' image, Picard's historical nerdery and crafted this elaborate fantasy that he either lost control of, or never had.

We haven't had that spirit here since 1969.

I did appreciate that not-Laris had a servo (the magic ballpoint pen that does everything) that's at least reminiscent of Gary Seven's.

On another topic, I don't see why a 21st-century doctor with a gurney and a defibrilator would do a better job than a 25th-century EMH with a biobed and a replicator. Yes, yes, I know, it's because they needed to get that character back into the story.

I guess that if the biobed alone couldn't help Elnor, the crew might not have faith in it helping Picard either, plus there's a good chance the Confederate La Sirena has no EMH and/or its power systems are still too unreliable. But yeah, the other thing.
