I read that wiki article on Musth and it says the glands in their heads get so engorged that they compress their eyes and cause intense pain. So they have extremely high levels of testosterone (140 times normal levels in extreme cases), suffering from extreme pain, and have impaired vision. They were lucky that feller was so seemingly calm and gentle.
