What exactly kills someone when they go over Niagra? Is it the impact on the water? Being crushed under it? Drowning because it's impossible to swim? Like, do people make it to the water only to suffer horrifically or do they just splat quickly and die on impact?

For those that miss the rocks, drowning most likely. The spray alone can be so strong that even when you're on the boat it can be tough to catch your breath if you're right in it. Imagine being in the choppy water with crazy currents and the spray in your face. I don't think "hitting the water from a height is like hitting cement" would really happen here, as the water is so churned up and aerated that there isn't that firm solid water surface that could cause an impact injury.

You don't fall into a pool of water, you fall onto a pile of jagged rocks with tons of water cascading down on you. You then bounce over the rocks into a pool of water with tremendous currents that will pull you under for 10s of minutes at a time.
