I’ve only ever used the free version. It’s suited my needs well enough in Boston. Maybe the limitations of the free one would be apparent in somewhere like NYC with more than a few lines

I would say that they just have to be able to occupy the same amount of space. So if the man was only just small enough to curl up into the volume of an F150. This probably wouldn't create F150s that you'd imagine when you see the word "man sized" but it would be fair I think.

In practice, the minimum liability insurance required in most states is way, way loving less than needed to cover even moderately bad medical expenses, and tons of people are driving around without even that.
