It primarily happened because Russia forced the advance by bringing in a large complement of reinforcements (both armor and troops, especially large numbers of mobiks) which allowed them to leverage numbers over what Ukraine had to defend in that region. Currently Ukraine also set up reinforcements to contain this advance near the captured town. In practice, Avdiivka itself is in no more danger than it was previously because their critical supply lines and southern defenses are much stronger and under no immediate threat. Reporting from Ukraine also said these gains can be reversed as long as force parity is maintained by Ukraine in the area.

Reminds me of that club fire in Russia a couple of years ago where over 100 people died. IIRC the ceiling was highly flammable, a band was using pyrotechnics and all but one emergency exists were intentionally locked. My feeling is that fire safety standards aren't very thoroughly enforced in Russia
