Declining and ever more disfunctional institutions is where I draw it as, after all they are what represents governmental strength. A lack of government as you describe is what happens well past a complete institutional collapse and is usually catastrophic/apocalyptic.

As the system loses legitimacy the Security Forces are going to demand more and more money. Eventually the Ruling class will either refuse, be unable to pay or the Security Forces get a better offer.
