Supposedly, Ukraine created an upgrade to increase the ammo storage protection in their T-84s, but I know nothing about it. Ammunition cook-offs happen in Abrams and Leopard 2s from time to time. I can't speak for the L2s, but I know that Germany has tried to mitigate the damage that cook-offs may do to the crews. The Abrams has a large number of fallback measures in order to make crew fatalities from an ammo cook-off be a freak occurrence. There are anecdotal stories of Iraqi crews of being struck, then being trapped inside the tank for different reasons while the ammo cooked off, then being able to later leave the tank with minor burns/injuries.

Tanks and other armored vehicles existing that do reasonably good jobs of protecting crews is in my opinion one of the real miracles of modern engineering. The problems are much less of an issue in the former scenario.

They also assumed, quite rightly at the time, that low profile and high (forward) speed were more important than armour that wouldn't protect against getting hit anyway, because armour tech had not kept up with penetrative power during that time period. The Leopard 1 was built under the same assumption and is a similarly dire death trap.

According to historical accounts, the Germans were mostly unimpressed the first time tanks appeared. I mean in a war that completely transformed all aspects of war from ground up, what is another piece of technology, especially one that is "just" a combination of existing pieces? They had already seen armored cars, artillery tractors, giant shells thrown by massive guns bursting in the air, and don't forget airplanes...
