First name being the same is probably preferable to last name (there's an example like that in my family tree, and no they were not actually related ), although that presumably isn't an issue in countries like the US where it is customary for women to take their husband's last name upon marriage.

I know 2 couples with the same first name: Pat and Pat, and Jo and Joe. When my mum got Spanish citizenship, she was offered the opportunity to very easily Spanish-ify her name, by (among other things), adding a second surname. Two of the options suggested were to add your mother's maiden name or to simply double your existing surname. Apparently it's quite common among certain groups of immigrants. Also not super uncommon among people of Spanish heritage either, naturally, given how common some surnames are.
